Know what the OSBI repository contains

Oklahoma State Bureau of Investigation history reports contain Oklahoma fingerprint-supported arrest and conviction information for serious misdemeanors and felonies. They are not conviction-only reports. Unreported arrests, missing fingerprint submissions or unusable prints can leave gaps. OSBI does not supply other states’ histories or driving records, and says it does not maintain juvenile criminal-history information unless the juvenile was certified as an adult.

A felony label beside an arrest charge describes the alleged offense at arrest; it is not itself a felony conviction. Read the disposition and the charge ultimately filed. For the actual judgment or pleadings, obtain the file from the court clerk identified by the case, rather than treating the repository response as a complete court file.

Submit a name search with usable identifiers

OSBI’s public request instructions require at least the subject’s full name and date of birth and allow up to three alias names per search. Include relevant previous or maiden names rather than assuming one spelling covers them. The FAQ says a signed release is not required for this public history request. The repository may be built from fingerprints, but the query you submit through CHIRP still matches by name.

Open CHIRP from OSBI’s official page for the electronic name-based request. The service requires an email address and payment by credit card or checking/savings account. Retain the response and its date. If a licensing or hiring organization specifies an authorized fingerprint process, use its instructions; a public name report is not automatically interchangeable with that process.

Use the fee for the submission date

As checked October 2, 2026, OSBI’s request page lists $15 for a name-based history and $19 for a fingerprint-based history. Its newer CHIRP notice announces that on November 1, 2026 those charges increase to $20 and $25 respectively. The state fingerprint option remains mail or in-person only. Budget against the date you will submit, not the date an older form or saved page was printed.

The DOC sex-offender and violent-offender database searches are separate add-ons listed at $2 each, or $4 for both. DOC maintains those databases even when OSBI takes the request. Select only the checks required and keep each result labeled; buying an extra registry search does not transform an Oklahoma history into a nationwide report.

Mail, fax and counter requests

For an offline name request, use the Criminal History Request Form and submit it to OSBI, 6600 N. Harvey, Oklahoma City, OK 73116, or fax it to 405-879-2503. Paid counter requests are accepted Monday through Friday, 9 a.m. to 4 p.m., excluding state holidays. Include a postage-paid return envelope with mail requests. The request page accepts cashier’s checks, money orders and credit cards, with cash only in person and credit cards only for fax requests. Personal checks are not accepted.

OSBI publishes a usual 10–15-minute wait for one in-person name check, 1–3 weeks for regular mail, and 2–5 business days for fax requests. These are service estimates, not guarantees for every record. Call the Criminal History Reporting Unit at 405-848-6724 if a mailed response has not arrived within 30 days, using the request date and payment details to identify it.

Send original fingerprints for identity-based matching

A fingerprint request requires the history request form, the applicable fingerprint-search fee and an original applicant fingerprint card. The card must be complete, signed and dated by both the printing officer and the person printed; do not fold, staple or alter it. Submit by mail or in person, never by fax. OSBI’s FAQ says to allow at least four weeks for a fingerprint search and that the reply is mailed.

Supply the missing court outcome

When the repository lacks a final disposition, obtain a certified court document and send it to OSBI, Attn: DSU Unit, 6600 N. Harvey Place, Oklahoma City, OK 73116. OSBI’s request page links its Update Your Criminal History instructions and form. It does not accept update faxes from individuals; even an accepted court or law-enforcement fax must be followed by a certified hard copy.

A completed deferred sentence may require a certified dismissal order to update the recorded disposition. OSBI explains that updating the plea and dismissal can still leave the arrest on the history. Complete removal or expungement requires a court order. Do not describe a missing sentence, dismissal update and full expungement as the same result.

Handle an incorrect charge or identity match

If the arrest charge is wrong, contact the arresting agency: it supplied the fingerprint-card information and can send reviewed corrections to OSBI. If someone used your identity, ask OSBI about its fingerprint-comparison procedure and Identity Theft Passport information. A similar name alone cannot establish that a returned history belongs to you or another person; retain the disputed entry and any court documents while the match is resolved.
